LexisNexis Coplogic Solutions
On July 15, 2019, the McCook Police Department began using LexisNexis Coplogic Solutions (e-Crash) for the purposes of completing accident reports. If you were involved in an accident within the boundaries of the Village of McCook, the responding officer should have provided you with a card containing specific information regarding your accident and how to access your report online.

How to Access Your Report
Follow these steps to retrieve your accident report or driver's exchange form online.

Locate Your Card
Find the information card provided by the responding officer at the scene of the accident. It contains the details you will need.

Visit the Portal
Click the "Access Your Report" button above to visit the LexisNexis Coplogic Solutions portal.

Enter Your Information
Use the report number and crash date from your card to look up your accident report or download the free driver's exchange form.

Download or Purchase
The electronic driver's exchange form is available at no cost. Full accident reports are available for purchase through the portal.
Illinois Motorist Report
Illinois statute requires that the driver of a vehicle involved in any manner in a crash shall, as soon as possible but not later than 10 days after the crash, complete an Illinois Motorist Report.
Drivers must have access to the Driver Information Exchange Application, the Report Number, and the Date of Crash in order to complete the Motorist Report.
Please note: It may take up to 24 hours for the State of Illinois to assign a control number to the traffic crash report and for the information to become available. This field may display "N/K" (not known) if the number has not yet been assigned.
